To proceed with a scheduling inquiry, please fill out this form (available on the website) and return it to the business office as soon as possible. The form may be submitted online, faxed (515-243-2763), mailed to St. John’s (600 Sixth Avenue, Des Moines, Iowa 50309) or a staff member may also take this information over the phone. The form is submitted to the pastoral staff for consideration. The couple will be notified of the decision by mail within 2-3 weeks of the application.
